    Abstract: 「藥物不良事件」(Adverse Drug Event)的發生,可能造成病人不同程度的傷害,甚至造成病人死亡,因此,如何預防及降低藥物不良事件的發生,以保障病人安全,是刻不容緩的事。尤其目前國內外醫療均以「病人安全」為首要目標,在探討提昇「病人安全」與「醫療品質」之前,必定要先降低醫院內部引發的「藥物不良事件」及「醫療錯誤」。根據國外研究指出,在住院病人中,平均約有6.8%的病人發生藥物不良事件,1993年的回顧報告中也指出美國每年大約有7000人,死於用藥疏失,相當於每845位住院病人中,就有一位死於用藥疏失。1995年 Leape LL在藥物不良事件研究中指出,住院病人的用藥錯誤(Medication Error),有11%發生在藥師調劑(Dispensing)階段。本研究旨在探討,如何將無線射頻辨識技術導入藥品調劑台,以降低藥品調劑疏失與提升病人用藥安全。藥師調劑藥品時,利用無線射頻識別技術自動進行病人、處方與藥品三方面之比對,並將比對過程與結果以電腦呈現、記錄,以期減少因人為疏忽引發之藥物不良事件、給藥錯誤與醫療糾紛,提高病人的用藥安全、創造科技化醫療環境。
    As many as 44,000 to 98,000 people die of medical errors in the United States each year. Of them, 9% of the deaths are attributable to medication error. The medication errors are account for approximately 6.8% of deaths relating to medical errors per 100 inpatients. This smart dispensing platform integrating Radio Frequency Identification (RFID) could assist pharmacist to monitor wrong drug, is therefore reduce dispensing errors and promote patient safety.
