    標題: 介入桌遊活動對社區老人的認知功能與健康生活品質 成效之研究
    Effect of table game intervention on cognitive function and healthy quality of life of the elderly in communities
    台灣面臨人口?化日?嚴重的趨勢,然而在老化的過程中因大腦組織的流失,導致認知功能產生退化的現象。藉由認知訓練有助於減緩認知功能退化,因此,本研究探討桌遊活動介入對認知功能與健康生活品質之成效。本研究以立意取樣方式,選出南部5個社區關懷據點65歲以上的老人為研究對象,於2018年10月及2019年1月由受過訓練的訪視員至社區關懷據點進行實驗前第一次的資料訪談,桌遊活動介入後,進行第二次後測資料訪談,扣除無效樣本32筆,研究樣本為165筆(實驗組76筆及對照組89筆),問卷回收率為83.8%。以卡方檢定分析社區老人之特性、健康狀況與認知功能是否進步、健康生活品質是否進步並以卡方檢定結果有顯著相關的因子為自變項,進行二元邏輯斯逐步複迴歸分析。桌遊活動介入(實驗組)的認知功能為進步是對照組的2.744倍(p=0.008);保險類別總數為二種以上的認知功能為進步是保險類別總數為一種以上的3.844倍(p=0.009)。桌遊活動介入(實驗組)的健康生活品質為進步是對照組的2.942倍(p=0.025);退休前職業為家管者的健康生活品質(EQ-5D)為進步是退休前職業為(農、林、漁、牧)業者的5.965倍(p=0.026);跳舞認知活動參與程度(實驗前後每週社區舉辦的跳舞活動總分鐘差距)每增加一分鐘,健康生活品質(EQ-5D)為進步是原來的1.003倍(p=0.034)。最高教育程度高中(職)畢(肄)的健康生活品質(EQ-VAS)為進步是不識字的0.080倍(p=0.015);目前婚姻狀態為已婚有偶的健康生活品質(EQ-VAS)為進步是已婚喪偶的3.050倍(p=0.014);參與社區舉辦的各項表演認知活動參與時間(c)每增加一分鐘,健康生活品質(EQ-VAS)為進步是原來的1.006倍(p=0.007)。本研究建議衛生主管機關及社區關懷據點單位,在設計社區老人課程內容時建議多安排相關桌遊活動、跳舞及表演的帶動等。增加活動時間在桌遊活動方面,不僅能促進老年人腦部的運作及增加社交互動等,達到預防及延緩失能、改善認知功能。而增加老年人跳舞、表演活動時間過程的中,藉由肢體的擺動,提高協調能力,且伴隨著音樂可以讓心情愉悅等,增加老年人自我肯定的價值並培養多元活動,提升健康生活品質。
    Taiwan is facing a growing trend of aging population.However, in the process of aging due to the loss of brain tissue, resulting in cognitive function degradation phenomenon. Cognitive training can help slow cognitive decline. Therefore, the purpose of the study was to explores effect of table game intervention on cognitive function and healthy quality of life of the elderly in communities.In this study, people aged 65 and over from five community care strongholds in the south were selected as the research object by purposive Sampling. The first pre-experiment data interviews were conducted in October 2018 and January 2019 by trained visitors to community care strongholds. table game activities after intervention, conduct a second post-test data interview. Excluding 32 invalid samples, the study sample was 165 (76 in the experimental group and 89 in the control group), and the questionnaire recovery rate was 83.8%. To analyze the Chi-square test of the elderly in the community, health status and cognitive function progress, health quality of life progress and the Chi-square test results have significantly related factors as a self-variant, bivariate logistic stepwise multiple regression analysis.The cognitive function of table game intervention (experimental group) was 2.744 times higher than that of the control group (p=0.008);The total number of insurance categories with more than two cognitive functions for progress is 3.844 times the total number of insurance categories of more than one (p=0.009). The healthy quality of life of the table game intervention (experimental group) was 2.942 times better than that of the control group (p=0.025);The healthy quality of life (EQ-5D) for pre-retirement occupations is 5.965 times higher for pre-retirement occupations (agriculture, forestry, fishing, animal husbandry) (p=0.026);The level of participation in dance cognitive activities (the difference in the total minutes of dance activities held in the community each week before and after the experiment) increased by 1.003 times(p=0.034) the improvement in the healthy quality of life (EQ-5D) for each minute of the experiment. The highest level of education is the healthy quality of life (EQ-VAS) in high school (vocational) and 0.080 times the progress of illiteracy (p=0.015);The current marital status is that the healthy quality of life of married couples (EQ-VAS) is 3.050 times that of married and widowed couples (p=0.014);For every additional minute of participation in performance awareness activities organized by the community(the total minute gap of various performances held in the community every week before and after the experiment), healthy quality of life (EQ-VAS) improved by 1.006 times as much as before (p=0.007).This study recommends that health authorities and community care units, in designing the contents of community courses for the elderly, should arrange more relevant table games, dance and performance promotion. Increased activity time in table games can not only promote the functioning of the brain of the elderly and increase social interaction, so as to prevent and delay inactiveness and improve cognitive function. And increase the elderly dance, performance activities in the process of time, by the body swing, improve coordination ability, and along with music can make the mood happy, increase the value of self-affirmation of the elderly and cultivate multi-activity, improve the healthy quality of life.
