急診護理人員檢傷專職團隊訓練之成效探討 -以南部某醫學中心為例

標題:	急診護理人員檢傷專職團隊訓練之成效探討 -以南部某醫學中心為例 The Effectiveness of Triage Training on the Emergency Nurses-Taking a Southern Taiwan Medical Center as an Example 背景：台灣急診自2010年始統一使用檢傷五級分類系統進行檢傷分類。目前台灣檢傷分級均由護理人員來執行，分級的正確與否，不僅影響病患的等候時間，更攸關急診病患的照護品質。本研究採橫斷式調查法，探討南部某醫學中心急診護理人員檢傷專職團隊訓練成效。研究目的: 一、急診護理人員基本屬性在檢傷分級能力的差異分析。二、檢傷專職團隊訓練對急診護理人員檢傷分類之成效。三、瞭解影響急診護理人員參與檢傷專職團隊之因素。方法：南部某醫學中心急診室護理人員為研究對象。立意取樣75人。採三組，每組25人分析比較，A組為檢傷專職團隊護理人員、B組為有檢傷經驗之非檢傷專職團隊護理人員、C組為非專職團隊人員且無檢傷經驗之護理人員，根據TTAS五級檢傷分類系統，給予人員採不記名方式進行檢傷分級能力測驗表，分析各屬性之相關性；並針對各組人員進行訪談，瞭解影響參與檢傷專職團隊之因素。本研究工具根據急診相關文獻及檢傷小組討論案例自擬而成「檢傷分級能力測試表」，並經由專家確認效度。研究結果：本研究共收案75位護理人員，結果發現檢傷小組測驗分數明顯高於其他兩組人員，故有受過檢傷專職團隊培訓人員檢傷能力有正成長。另急診護理人員之護理年資、檢傷年資、教育程度高及有檢傷訓練課程者，檢傷分級能力均顯著高於較低者。而性別、年齡、專業證照及能力進階統計上無顯著差異；最後參與團隊原因，急診護理人員認為檢傷分類是急診特性，想要挑戰並且勝任此工作，以及主管單位給予的福利，讓人員加入檢傷團隊意願增高。而工作人員間的溝通不夠完善，其為最大的壓力源。?結論：同樣使用TTAS系統下，受過檢傷專職團隊訓練之成員檢傷分級能力會優於其他護理人員，因此透過檢傷專職團隊訓練更能夠效提升急診護理人員檢傷分類正確性，進而提升醫療品質。 Background: In Taiwan, computer assisted Taiwan Triage and Acuity Scale (TTAS) was applicated completely since Jan 1, 2010. Currently, the triage in emergent department was implemented by nurses. The precision of triage might affect the waiting time and the quality of treatment. This is a cohort study to investigate whether the triage-training team held by a medical center can improve the triage ability. Method: 75 emergent department nurses classified as three different groups including (A) triage-training group (B) triage-experienced nurses (C) triage-unexperienced nurses completed the triage questionnaire according to TTAS. The survey was anonymous and the questionnaire was constructed according to literature review, clinical situations and the discussions from triage meeting. The content was also validated by experts of triage. The score was calculated and compared with the demographic data in the three groups. Result: Among the three groups, the score in the triage-training group was higher than the other 2 groups. Besides, the score was higher in triage-experienced nurse, those with higher educational degree and ICU trained nurses (p<0.01). The score was not associated with the age, seniority, professional license. Conclusion: With the TTAS assisted triage, nurses in the triage-training group showed better performance than the other two groups .Consequently, triage training can effectively enhance the accuracy of triage, which improves medical quality in the lone run.
