從全民健保實施以來,醫療院所之間的競爭,所以給了醫院管理者重大的挑戰,醫療院所必須透過各種不同的品質指標系統來維持及改善院內之品質。所以醫策會研發了臺灣臨床成效指標計畫(Taiwan Clinical Performance Indicator, TCPI)。本研究以任務-科技配適及計畫行為理論為理論基礎,建立並驗證品管人員對TCPI系統之持續使用意圖及績效。本研究採問卷調查法,以使用TCPI之全台101家醫療院所為研究對象,共回收了有效問卷102份,再以結構方程模式之最小平方法來驗證假說。研究結果發現,科技特性對任務-科技配適有顯著性;任務-科技配適對持續使用意圖及績效影響有顯著性;主觀規範及制度信任對持續使用意圖有顯著性;結構保證對制度信任有顯著性。從實務的觀點,瞭解使用者對臺灣臨床成效指標(TCPI)的持續使用意圖及績效影響的影響因素,可幫助醫策會與其他研究者瞭解及修正臺灣臨床成效指標(TCPI),讓TCPI能夠更符合醫療院所的需求,並增加學術界瞭解臺灣臨床成效指標(TCPI)之重要參考依據。 Since the rollout of National Health Insurance, the competition between medical institutions has generated a major challenge to hospital administrators. Medical institutions must maintain and improve medical quality through implementing various quality indicator systems.Given the circumstances, Joint Commission of Taiwan (JCT) has developed the Taiwan Clinical Performance Indicator (TCPI) that. Therefore, based on the theory of Task-Technology Fit and Planned Behavior, this study establishes and verifies the influence of quality manager on the intention and performance of the TCPI system.
In this study, the questionnaire survey was adopted to study 101 medical institutions throughout Taiwan that currently are implementing TCPI. A total of 102 valid questionnaires were collected, and the hypothesis was verified by the least square method of structural equation model.
The research results show that the characteristics of science and technology are significant for task-technology adaptation; task-technical adaptation is significant for intentions and performance; subjective norms and institutional trust are significant for intentions of use; structural guarantees are significant for institutional trust. From a practical point of view, recognizing the influence factor of study subjects on the adoption experiences of Taiwan's Clinical Performance Indicators (TCPI) can help the Medical Council and other researchers keep refining the Taiwan Clinical Outcome Indicators (TCPI) to be a better fit to medical institutions’ needs, and to be an important reference for academics grasping the purpose of Taiwan's Clinical Performance Indicators (TCPI).
