摘要: | 智慧醫院乃是整合雲端科技及物聯網等資訊科技,以建構出創新的臨床應用與服務,而直接與病人臨床照護相關之系統即智慧病房。因此智慧病房是創新科技在醫療機構採用的典範,也是近年來重要的醫療資訊推動方案,然而智慧病房之發展除完整規劃與初期導入階段之成功外,在導入後階段中,更期望透過持續使用的評估,來確立科技資源之有效應用。
本研究以南部某醫學中心之智慧病房為研究標的,並依據資訊系統接受後持續使用模式為理論基礎,透過問卷普查、深度訪談、實地觀摩和相關資料收集方式,探討影響護理人員持續使用智慧病房因素,歸納量化與質化研究之資料分析結果: (1)本研究所提出的研究模型經實證資料檢測後,具有良好的信度與效度,可適用於衡量影響護理人員持續使用智慧病房相關因素(2)影響護理人員持續使用智慧病房因素歸納如下: 護理人員的確認顯著影響知覺有用性、護理人員的確認、知覺有用性顯著影響滿意度、護理人員的知覺有用、習慣與科技壓力顯著影響持續使用。
研究結果可供醫療機構發展智慧病房歷程中,持續關注專業人員在導入後階段影響其持續使用行為之重要因素,以作為持續改善系統發展,滿足專業人員任務需求與系統應用之效益。 Smart hospitals utilize integrated information technologies such as cloud technology and the Internet of Things to develop innovative clinical applications and services such as smart wards, which are systems with direct relevance to the patients' clinical care. Hence, smart wards have become an example of the application of innovative technology in medical institutions, and have become important in the promotion of medical informatics in recent years. However, the development of smart wards should not only includeinitiation adoption phase and a successful implementation phase, but also continuous assessments during the post-adoptionphase, so as to ensure the effective application of technological resources. On this basis, the smart ward of a medical center in Southern Taiwan was selected as the subject of this study, and the theoretical framework of this study was based on the post-acceptance model of Information system continuance. Questionnaire surveys, in-depth interviews, on-site observations, and other relevant methods of data collection were conducted to investigate the factors affecting the clinical nurses' continuous utilization of the smart ward.
The case study approach was adopted in this study. Data collection was conducted through quantitative questionnaire surveys and qualitative in-depth interviews, so as to investigate the factors affecting the nurses' continuous utilization of the smart ward, as well as the technological stress that they experienced. The smart ward that has been utilized for two years in a case hospital was the subject of research, and questionnaires were administered to its main users, that is, the clinical nurses, so as to understand the important factors affecting their satisfaction and continuous utilization. Qualitative in-depth interviews were conducted with nine smart ward users to identify the relevant important factors influencing their utilization satisfaction and continuous utilization of the smart ward. Important secondary data were also used for cross verification. A consolidation of the quantitative and qualitative data analysis results revealed several findings. (1) The research model proposed in this study demonstrated a reliability and validity measure of the relevant factors affecting the nurses' continuous utilization of smart wards. (2) The results revealed that smart ward continuance (R2 =.500) was mainly affected by perceived usefulness, techno-stress, individual habits, and user satisfaction was affected by confirmation and perceived usefulness of smart ward,whereas perceived usefulness was affected by confirmation.
Since the factors affecting the continuous utilization of smart wards by clinical professionals can be constantly monitored, the results of this study can serve as a reference for improving the development of smart ward in medical institutions during the post-adoptionphase, such that the task requirements of professional can be met and the effectiveness of the system's application can be achieved. |
