動機與目的：隨著臺灣長期牙醫醫療服務的進步與整體口腔健康狀況的改善，牙醫醫療服務也逐漸面臨服務模式的創新與整合，乃興起發展口腔與牙醫醫院之議題。然而，在與西醫醫療機構經營相近的背景中，投資發展口腔與牙醫醫院的可行性乃出現多元的意見。本研究乃應用社會判斷理論探討牙醫領域利害關係人的意見與看法，以評估當前牙醫醫療現場與社會環境發展口腔與牙醫醫院接受情形與決策影響因素。
研究方法：本研究分三個部分。首先舉辦專家焦點團體會議，會議於北中南東4場人數共為198人，經內容分析彙整出3個決策參考變數，包括「產業前景」、「經濟利益」、「社會責任」。其次，採用情境式問卷調查法，以三個決策參考變數為基礎，透過社會判斷理論指導設計出一個包含十五個情境的結構化問卷，以牙醫領域利害關係人為研究對象，採用立意取樣法，共計發放310份問卷，回收樣本111份，經Policy PC軟體計算個案十五個情境之一致性後作為納入分析的依據，有效分析樣本共為80人。其三依據社會判斷分析結果進行情境分析。
研究結果：牙醫領域專家之利害關係人對臺灣興建口腔與牙醫醫院之接受度，其判斷變數之重要性依序為「產業前景」(權重係數為34.7%)>「經濟利益」(權重係數為33.4%)>「社會責任」(權重係數為31.9%)，三者的圖形關係皆為線性正相關型態。
研究結論：整體而言，未來興建口腔與牙醫醫院時，需同時考量「社會責任」、「經濟利益」與「產業前景」，且三者的重要性新近。本研究建議後續探討興建口腔與牙醫醫院時，應同時納入政策制訂者與服務提供者等多元的牙醫領域利害關係人一同合作與規劃配套措施。本研究亦依據研究提出相關政策與管理建議，以供後續各界參考之用。 Motivation and purpose: With the long-term advancement of dental medical services and the overall improvement of population oral health in Taiwan, dental and oral medical services are gradually facing innovation and integration of service model, as well as the need for the construction of oral and dental hospitals. However, the similar industrial context of medical service makes the discussion of developing dental and oral hospitals controversial. Thus, the research propose is to explore the judgment and cognition of developing the oral and dental hospitals in Taiwan with applying social judgment theory
Methods: The research can be divided into three parts. First part, we held four expert focus groups (with 198 participants) in the representative area of Taiwan. With content analysis, we proposed three decision-making reference variables including “future industrial prospects”, “current economic benefits” and “reliable social responsibility”. Furthermore, a purposive sampling survey was made by the questionnaire of a fifteen vignettes which were developed by the focus group based on the guidance of social judgment theory. Of a total of 310 questionnaires delivered, 111 participants finished the survey. With the consistency analysis of the fifteen vignettes by case was made based on the cut-point of R2 value above 0.7, 80 participants was finally validated for the further analysis. Finally, we took a scenario analysis based on the results of the social judgment analysis.
Research results: With the social judgment analysis, the cognition of stakeholders in the field of dental industry on developing the oral and dental hospitals in Taiwan can be concluded three decision-making reference variables. The relative importance of three variables comes by “future industrial prospects” (with the relative weight value of 34.7%) > “current economic benefits” (with the relative weight value of 33.4%) > “reliable social responsibility” (with the relative weight value of 31.9%). The graphical pattern of the three variables exhibit a positive linear correlation.
Conclusion: On the whole, Discussions in the future construction of dental and dental hospitals, it is necessary to consider the three decision-making reference variables concurrently, and showing the similar relative importance. We suggest that a sufficient and sincere experts’ discussion and collaboration with various stakeholders should be put into consideration. Finally, we provide several suggestions for further research and policy making.
