    Title: 影響護理人員持續使用行動護理資訊系統與績效之研究

    Abstract: 近年來,醫療與健康資訊系統的發展是政府與醫療產業積極推動的重要策略,其中「行動醫療」對改善醫療品質、效率,以及降低醫療疏失有顯著成效。而國內醫療院所對於行動醫療之應用,大多是從行動護理資訊系統的發展為起始,主要是期望藉由行動資訊科技的應用,來提升組織與個人績效,然而組織導入行動資訊科技來改善績效之同時,對人員的原工作流程與臨床照護是一大衝擊,系統可能在採用過程中產生實務不契合或衝突問題,甚至影響此創新科技應用之預期成效,而此種問題通常在採用初期不易被鑑別,需進一步探討系統導入後階段影響持續使與績效評估,來進行問題鑑別與改善,然而過去較少探討此創新科技發展後階段影響人員持續使用及使用績效的相關研究。因此本研究目的為:探討影響護理人員對於行動護理資訊系統持續使用的關鍵因素以及持續使用的個人績效。本研究以「系統持續使用的後接受模式」為理論基礎,整合「科技準備度」與「習慣」之影響變數,以發展研究架構與研究假說,針對已發展及持續使用行動護理資訊系統達五年以上之南部某區域教學醫院,為研究範圍,以使用行動護理資訊系統達6個月以上之臨床護理人員為對象,進行問卷調查,共發放238份問卷,回收有效問卷219份,有效問卷回收率為92.02%。研究資料以AMOS統計軟體進行結構方程模式分析,並以整體模式配適度之契合度指標進行研究模式驗證,整體模式配適度指標均達標準。且8項研究假說均獲支持。研究發現:(1)本研究所發展之研究模型經實證資料檢測後,具有良好的信效度,顯示本研究所發展之研究模型,可適用於衡量影響臨床護理人員持續使用行動護理資訊系統與個人績效;(2)影響臨床護理人員持續使用行動護理資訊系統與績效因素,歸納如下:臨床護理人員的確認(0.382***)對認知有用性(R2 =0.505)具顯著影響;臨床護理人員的確認(0.710***)、認知有用性(0.413***)與科技準備度(0.138*)對滿意度(R2 =0.606)有顯著影響;臨床護理人員的認知有用性(0.325***)、習慣(0.405***)及滿意度(0.260**)對持續使用(R2 =0.651)有顯著影響;臨床護理人員的持續使用(0.810***)對個人績效(R2 =0.656)有顯著影響,整體模式解釋力達65.5%。(3)臨床護理人員對認知持續使用行動護理資訊系統所產生之個人績效評量中,認為使用行動護理資訊系統能夠有效改善臨床護理人員與醫療團隊之訊息交換(Mean=3.94)、與提升工作整體性(Mean=3.94)、並能更有效率的照護病人(Mean=3.93)及提昇病患照護品質(Mean=0.92)為排序前三項。研究結果可提供醫療機構發展行動護理資訊系統歷程中,持續關注專業人員在導入後階段影響其持續使用行為之因素與個人績效,以作為持續改善系統發展,滿足專業人員任務需求與系統發展之整體效益。
    This study aims at investigating factors affecting the clinical nurse’s continuance of using mobile nursing information system(MNIS) and individual performance from the perspective of a post-acceptance model of information system continuance. Survey research recruited nursing staffs as subjects from selected case hospital. A total of 238 questionnaires were sent out and 219 copies were obtained, indicating a valid response rate of 92.02 %. The data were then analyzed by the SPSS 18.0 and AMOS 18.0.The results indicated that clinical nurse’s confirmation (0.382***) had a significant impact on perceived usefulness (R2 = 0.505). Clinical nurse’s confirmation (0.710***), perceived usefulness (0.413***) and technology readiness (0.138**) significantly influencing clinical nurse’s satisfaction (R2=0.606). In addition, perceived usefulness (0.325 ** *), habits (0.405***) and satisfaction (0.260**) had a significant impact on the continued use (R2 =0.651). The clinical nurses continued use (0.810 ***) had a significant impact on individual performance (R2 = 0.656), accounting for 65.6% of total explained variance.Our study found that using MNIS has a significantly positive effect on individual performance. The respondents considered MNIS performance regarding: (1)improving message exchanges between healthcare professionals (Mean = 3.94); (2)enhance the work integrity (Mean = 3.94); (3) increasing efficiency of patient care duties (Mean = 3.93). Therefore, the development of MNIS can help nursing staffs in improving efficiency, effectiveness, and quality of nursing duties.These results can provide hospitals with factors affecting the continued use of MNIS and individual performance as MNIS becomes popularized in order to enhance its application and effectiveness. Furthermore, the results can enable managers to identify the factors that influence user evaluations regarding the effectiveness of MNIS and serve as a reference for the development and maintenance of future systems.
