    摘要: 近年來，行政院衛生署為加速醫師及病患診療決策時間、減少重覆性檢查及檢驗， 無不積極推動電子病歷交換，以促進院際間病歷互通整合來提升醫療資源運用效能。然 而，電子病歷的價值重點不應僅限於技術面，而是如何讓醫師使用電子病歷交換，才能 使其效益得以有效發揮；回顧過去相關文獻，主要集中於技術面議題的探討，對於電子 病歷行為面的研究非常有限。有鑑於此，本研究以解構式計畫行為理論為基礎，結合 Gefen et al. (2002)所提信任與風險中介關係的理論觀點，將制度信任及知覺風險視為電 子病歷交換意圖的重要影響因素之一，用以探討影響醫師對於電子病歷交換意圖的行為 模式。因此，本研究採用實證研究方法，以全國醫療影像交換中心網站所公告的提供電 子病歷交換的醫院名單為樣本來源，採系統抽樣方式針對醫師進行問卷調查，再以結構 方程模式進行資料分析，來驗證研究模式變數間之因果關係，藉以評估與驗證此整合模 式以及影響因素之關係；接著並實際選擇三家之醫院進行深度訪談，以期了解先前問卷 分析結果所代表之深度意涵。期望本研究之成果，能作為中央衛生主管機關、醫院與醫 療資訊業對於電子病歷交換執行與管理策略之參考，並增益學術界對於電子病歷之成功 與持續使用衡量之相關研究。
    This paper explains physicians’ acceptance, in terms of usage intentions, of the electronic medical record records (EMR) exchange. Although several prior researches have focused on the factors that impact on the adoption or use of information technology or Internet, there is limited empirical work which simultaneously captures the success factors that help physicians to adopt electronic medical record (EMR) exchange. Our study combined institutional trust, perceived risk and the Decomposed Theory of Planned Behavior (DTPB) model to identify factors that affect a physicians’ willingness to use EMR exchange. Based on this study contribution, healthcare policy makers can promote EMR exchange and hospitals can identify desirable strategies that will form regulations to meet the dynamic challenges in the healthcare industry.
