    Abstract: 得舒飲食是近年來廣為宣導的高血壓防治飲食。本研究之目的是依“得舒健康食譜開發與降血壓效益評估研究”之結果,進一步以得舒量表(DASH score)探討得舒飲食介入對高血壓前期及第一期高血壓受試者之血壓及血液生化值的影響。實驗設計是將54位高血壓前期及第一型高血壓受試者隨機分成得舒組和控制組,提供得舒或一般午餐,一星期供應五天,為期八週。在實驗第1週(前測)、第8週(後測)及第16週(追蹤)進行營養知識、飲食行為問卷及24小時飲食回憶評估,同時以得舒量表(DASH score)推估受試者的飲食對血壓及血液生化表現的影響。結果顯示,在血壓方面,前測平日之DASH score增加一分,收縮壓會降低1.90±0.69 mmHg(P < 0.05),前測假日之DASH score增加一分,舒張壓會降低1.51±0.62 mmHg(P < 0.05)。血管硬化指數方面,前測平日、假日、平日一餐、後測假日之DASH score增加一分,血管硬化指數分別降低0.13±0.05(P < 0.01)、0.12±0.05(P < 0.05)、0.16±0.07(P < 0.05)及0.22±0.06個單位(P < 0.001)。此外,後測假日之DASH score增加一分,會使三酸甘油酯降低10.81±5.19 mg/dl(P < 0.05)、動脈硬化指數降低0.20±0.08個單位(P < 0.01)。 本研究結果顯示DASH score可以作為得舒飲食餐飲設計的參考依據,改善血壓、血脂的飲食模式。
    The Dietary Approaches to Stop Hypertension (DASH) diet is broadly recommended for the prevention and treatment of Hypertension. The purpose of this study is based on the study results of “Menu Development of DASH Healthy Diet and Evaluation on Function of Lowering Blood Pressure”, to further investigate the effect the diet on the blood pressure and blood biochemical of pre- and stage 1 hypertension patients, by using DASH score. A randomized clinical trial was undertaken in 54 pre- and stage 1 hypertensive patients. Participants were randomly assigned to control group or DASH healthy diet group, and carried out diet intervention by offer DASH or general lunch, five days a week and a period of eight weeks. The KPQ and 24 hours dietary recall were assessed on week1 (pre-test), week8 (post-test)¸ and week16 ( follow-up). The results were then correlated with DASH score to estimate the effect of DASH diet on blood pressure and blood biochemical. Blood pressure – the results showed that for pre-test weekday, one point increase of DASH score resulted in systolic blood pressure drop of 1.90 ± 0.69mmHg(P<0.05); on pre-test weekend, one point increase of DASH score resulted in diastolic blood pressure drop of 1.51 ± 0.62 mmHg(P<0.05). Vascular sclerosis – on pre-test weekday, weekend, meal on weekday and post-test weekend; one point increase of DASH score showed reduction in SI ( CHOL/HDL-C ): 0.13±0.05(P<0.01), 0.12±0.05(P<0.05), 0.16±0.07(P<0.05)and 0.22±0.06(P<0.001)units respectively. Also, on Post-test weekend , one point increase of DASH score led to reduce in triglyceride level 10.81 ± 5.19 mg / dl (P<0.05); SI ( TG/HDL-C ) 0.20 ± 0.08(P<0.01)The results of this study suggest that DASH score could be the reference for modeling DASH healthy diet to prevent blood pressure and blood lipid.
