    摘要: 電子病歷在醫療活動上,主要是用以解決傳統紙本病歷無法多人共享以及空間儲存的問題。因此,為了因應國內醫療政策、健保給付規定、醫院評鑑以及消費者知的權利之需求,讓電子病歴的實施成為各家醫院的重要基礎建設之一。然而,目前卻缺乏一套客觀、有效的使用者滿意度評估方法,致使電子病歷資訊系統難以展現其真正的價值。有鑑於此,本研究以差異理論觀點為基礎,整理過去相關文獻,以DeLone and McLean(2003)的資訊系統成功模型中的品質構面及資訊人員專業能力為自變數來衡量對於電子病歷資訊系統之使用者滿意度影響,期望說明造成使用者滿意度差異的原因所在,藉由比較不同使用者注重程度與實際感受間的差異。因此,本研究目的旨在探討在不同使用者(醫師及病歷管理人員)之間影響電子病歷資訊系統滿意度的因素為何?在模式驗證方面,本研究是以醫師及病歷管理人員為研究對象,採用問卷調查法,針對一家醫學中心、兩家區域教學醫院之醫師與病管人員之使用者進行研究調查,回收有效樣本為111份,分別為醫師48份,病管人員63份,整體有效回收率為79.29%。問卷經由變異數分析與迴歸分析進行研究假說後,獲得以下之結論:(1)醫師群之使用者,資訊品質注重與感受之間的差異,影響使用者滿意度最大;病管群之使用者,IT人員專業能力注重與感受之間的差異,影響使用者滿意度最大;整體群之使用者,IT人員專業能力注重與感受之間的差異,影響使用者滿意度最大。(2)不同使用者族群,因工作性質不同,而造成使用者滿意度的各品質構面也會有所不同。(3)經由調節廻歸分析中發現,各品質構面是影響整體群之使用者滿意度的重要因素。最後,本研究結果將可提供實務界管理上的建議,並供後續研究學者繼續深入探討與參考。
    This study aims at combining the information system success model and discrepancy theory to evaluate user's satisfaction. By comparing user’s gap between want and have, we can understand the influencing factors of user satisfaction on information systems among different user groups (i.e. the physicians and medical records management staffs). The independent variables are information quality, system quality, service quality, and information system specialist’s skill and dependent variable is user satisfaction on Electronic Medical Records (EMR) information systems. The research methodology in this study is based on empirical data collected through a questionnaire survey of 3 hospital staff. There were 111 usable responses (response rate is 79.29%) from the users of the EMR information systems. ANOVA and regression analysis techniques method were employed to test the research hypotheses. The results indicated that, for physicians groups, information quality affect user satisfaction the most; whereas for medical record management group, information system specialist’s skill affects user satisfaction the most. The results can guide us to devote limited resources to improve user satisfaction with the Electronic Medical Records (EMR) information systems. The implications of these findings are discussed, and managerial guidelines are presented.
