Abstract:	本研究除分析系統中不安全的狀況之外，嘗試以導致不安全的行為的個人因素，一併在失誤樹分析中作澈底的系統分析。無論從失誤樹的圖形分析或失誤樹所代表的布林代數式，皆可獲致一個重要的觀念，即管理與決策的失當，以及個人因素為意外事故發生原因中最重要的因素。不安全的狀況雖亦為主因，但重要性不及管理與決策的失當及個人因素。從兩件實際發生的工業災變，有關意外事故發生的原因的理論與本研究以失誤樹分析的結果完全吻合。
