台灣目前已進入高齡化社會,老人之照護變成一個重要課題,長照機構因此如春筍而立。長照機構為達完善之服務,除了提供優美之環境、營養餐飲及優質之醫療服務外,完備之藥事照護也是重要一環。但觀看目前在台灣之長照機構,藥師之參與才在起步階段,而且參與的意願不高。若能提升藥師參與長照機構之意願與機會,除了可提升長照機構對住民之服務品質外,也可增加藥師之收入,而達長照機構、藥師及住民三贏之局面。
本研究以長照機構、藥師及住民為三大主題,使用懷德術方法進行增進三者之間合作績效影響因素之專家訪談,擬定出住民家屬、長照機構、健保藥局、認知績效、照護合作效能及同意報酬等六個影響健保藥局藥師介入長照機構進行藥事服務之合作模式因素構面,再以問卷調查方式針對長照機構及藥師做問卷,探討出能增加藥師參與長照機構藥事服務合作意願之影響因素,以擴展藥師專業之服務及提升長照機構藥事服務之品質。結果顯示長照機構的專業照護若以個別單一項目進行服務時,目前均無法獲得合宜的報酬收入,只有聯合多項專業的照護才可得到住民家屬之同意報酬正向之關係。因此,對於推動長期照護機構之合作照護與加強各項專業照護之服務深度有積極與正向的未來影響。 Nowadays, Taiwan has became an aging society. Taking care of elderly people is turning into important work; therefore, many long-term care organizations (LTCOs) have rapidly grown up. The LTCOs provide beautiful environments, nutritional diets and medical services in order to have a good health care system. Besides these, the high quality of pharmaceutical services is also an important item. But in Taiwan, we observed that pharmacists participated in the pharmaceutical services of the LTCOs only in the beginning stages, and they have shown less willingness to continue on participating in this work. If we could enhance the pharmacists’ participation in the LTCOs for pharmaceutical services, they could provide high quality health care services to long-term care residents. Through this way, it may not only increase the revenue for pharmacists, but may also help the LTCOs and long-term care residents obtain the advantages of such a system.
There are three subjects in this study:the LTCOs, pharmacists, and the long-term care residents by using the Dephi method. Through this methodology, we can better understand the influencing factors that affect the cooperation between the three subjects. The interview questionnaire is based on six main areas of potential influence:family members of residents;LTCOs;community pharmacies;cognitive performance;cooperative care;and payment. The questionnaire is designed to investigate how to increase pharmacists’ willingness to participate in long-term care organizations, as well as to inform them on how to expand their professional service in order to enhance the quality of medical services in long-term care organizations. The results of the questionnaire indicate that, if LTCOs only provide one service, they cannot earn appropriate profit. Only cooperation with more professional experts can positively lead to the residents paying more money for these services, with the result that long-term care organizations will definitely be able to provide an excellent quality of health care with the promise of continued high quality service.
