| 摘要: | 在衛生署資料中顯示 2006年死亡人數為9,690人，占總死亡人數之7.2%，死亡率達每十萬人口42.5人。在台灣地區糖尿病死亡率為每十萬人有34.9人，由此推估台灣地區約有近百萬名的糖尿病患者。筆者希望透過研究醫院推行之糖尿病照護模式，了解糖尿病患有無參加共同照護計畫，對照護過程品質及病患結果成效之差異性，提供更好醫療照護服務，期盼研究結果能作為醫院提升糖尿病照護品質之參考。
本研究目的：一、探討糖尿病照護網計劃，對糖尿病病患之血糖值、糖尿病合併症及住院率的影響。二、探討糖尿病照護網計劃對糖尿病患的自我照顧認知能力的影響。三、分析影響糖尿病照護網計劃成效之相關因素。
以苗栗縣某區域教學醫院門診糖尿病病患為研究之母群體。採類實驗性之研究設計法，以「有加入共同照護網」與「未加入共同照護網」兩組，進行照護網品質及成效比較。
研究結果及結論：血糖平均值有加入共同照護網與未加入158.27VS179.52；認知程度加入共同照護網平均為4.93 VS3.76；急診就醫次數及住院狀況也較未加入照護網者少。本研究結果呈現顯示照護網推展有達到成果，接受共同照護網的病患，其血糖值較未加入照護網者低、對疾病自我認知較好，表示經共同網照護過程有效影響品質結果，而達到具體成效。
